A well-contoured neck is the sine qua non of an attractive, youthful, physically fit and sensual appearance. The delicate skin of the neck is one of the first places visible signs of aging appear.  In the age of selfies, facetime and a generation constantly on display on social media, a distinctly defined, sleek, smooth neck with a strong jawline is emerging as one of the most sought after attributes for both men and women.

For decades, the neck has been one of first tell tale signs of aging. The loathsome double chin plagues many even from a young age. The turkey gobbler neck is one of the most popular concerns bringing men into plastic surgeons’ offices. Weight fluctuations, repetitive muscle pull and thinning of our skin over time all contribute to visible changes. Yet the neck is not getting the media attention or acknowledgement from the general public it deserves. A neck lift, also known as a platysmaplasty, is a powerful procedure that can reverse the effects of aging to restore a more youthful appearance of the neck. When performed properly, the result is natural and helps you appear as a rejuvenated version of you. The effect should be largely imperceptible, even to people who have known you for years.

 

Dr. Deniz Basci’s elevated neck lift consistently delivers reliable first-class results. There are a number of different techniques she uses depending on what your neck needs. The goal is to reverse tell-tale signs of facial aging and bring back the neck tissues to a more youthful configuration. A elevated neck lift reunites the attenuated loosened platysma muscles in the neck. Once the contours of the neck are smoothed and tightened by repositioning the deep tissues, the skin envelope is carefully re-draped and the excess precisely trimmed. The key to a natural result is re-suspending the deep tissues from the appropriate height at the most flattering angle and placing absolutely no tension on the skin. In the operating room, Dr. Basci has full visualization of the facial tissues, and maximum possible control to safely execute the necessary maneuvers.

 

The elevated neck lift incisions are well hidden along the hairline, behind the ear and under the chin. Typically, the scars heal nearly imperceptibly because of Dr. Basci's technique. Each procedure is custom tailored to the individual like a fine Italian suit. No two procedures are exactly the same.

 

In some cases fat grafting to the angle of the jaw or a chin implant may be indicated to enhance your features and help elongate the appearance of your jawline and neck. Often a weak chin contributes to the prominent appearance of the nose as well as the short and obtuse appearance of the neck.

Preparing for Surgery

Prior to any surgery, Dr. Basci will review your general health history, surgical history and current medications. Most patients will be asked to stop taking any medications or supplements that increase their risk of bleeding 2 weeks before surgery and remain off of them for the 2 weeks following surgery. Make sure to check with Dr. Basci and  your primary prescribing physician before stopping any medications.

        

 It is important to prime your body with a healthy balanced diet in the months leading up to surgery. Make sure you have appropriate protein intake, as protein is vital for healing. You may need to supplement your diet with protein shakes or bars for 3-6 weeks after surgery as your body works overtime to mend all of the tissues in your surgical site.

 

Preventing sun damage with routine use of SPF > 40 in the months leading up to and following your finesse facelift will protect and enhance your result. Presenting a polished canvas for your new neck with radiant glowing skin will exponentially improve your youthful appearance. Dr. Basci also recommends a high quality skincare regimen for 2-3 months prior to surgery, depending on the condition of your skin, and will talk to you about your existing routine. In cases of extensive sun damage, laser resurfacing may be recommended to help smooth any texture irregularities and remove pigmented lesions contributing to the appearance of coarse, dull, aged skin. Pre-surgery and post-surgery maintenance photo-facials can diminish the appearance of pigmented lesions, prominent blood vessels, and will help enhance and preserve your neck lift result.

During Surgery

Every individual has different aesthetic desires and problem areas in the neck they seek to fine-tune. Dr Deniz Basci has advanced training and expertise in facelift surgery and will design a customized treatment plan based on your needs. She uses either a platysmaplasty technique that tightens the muscle underlying the skin of the neck to help support and contour the final result. In some instances the neck lift is combined with liposuction or partial salivary gland removal depending on your natural anatomy and tissue quality. Either technique is capable of delivering dramatic, long-lasting and natural results.

 

Typically incisions are made behind the ear extending down along the hairline and under the chin. The skin is then elevated allowing access to the underlying neck tissues. These are assessed and elegantly re-suspended to highlight and define the jawline. Excess fatty volume is removed below the jawline and under the chin to emphasize the elongated smooth contour. Care is taking to preserve an even layer of plush youthful padding. With the deep neck tissues in their new youthful position, the skin is delicately re-draped and the excess removed.

 

Dr. Basci uses meticulous tension free suturing techniques to help incisions heal in the best way your body produces scar. A drain is placed on one side of the neck followed by a soft cotton wrap to provide gentle pressure and minimize swelling. The surgical dressing is ordinarily removed the morning after surgery and the drain in 5-7 days.

Routine Recovery

Just as every individual procedure is distinct, so is every personal recovery.  Most patients will go home the same day as their procedure. You will need a family member, friend or nurse to stay with you for at least 2 days after that, although 1 week is recommended for your comfort.

 

You will have one drain in the neck that will need to be emptied and monitored twice daily. Drains are removed whenever the fluid output has decreased to less than 20ml in 24 hours; usually in 5-7 days. You may shower after your drains are removed. Avoid curling irons, intensely hot blow dryers, and chemically coloring your hair for 1-2 weeks after surgery, as your scalp may be numb in certain areas. This is to avoid inadvertently burning your scalp.  

You will wear a soft compressive neck wrap 24 hours a day for the first two weeks to help the skin adhere to the newly contoured underlying tissue. During this time you should also minimize head turning and extreme neck flexion or extension. After two weeks, you may wear the neck wrap only at night or when you are at home. Typically patients feel tightness in their neck, this will gradually improve along with swelling and healing.

 

Any non-dissolving sutures will be removed in the office at your first a post-operative visit. You will need to apply a thin layer of ointment to your incisions once a day until they are fully healed. Talk to Dr. Basci about when it is safe to begin lymphatic neck massages and silicone scar treatment.

 

You should supplement your regular balanced diet with protein shakes or bars for 3-6 weeks after surgery to help improve healing.  This is not the time for diets or nutritional restriction. Your body needs vital amino acids and building blocks for healing.

 

Dr. Basci recommends you sleep with your head propped up on several pillows or semi-upright in a recliner for the first 1-2 weeks after surgery to minimize facial swelling. You should expect to be moderately swollen during the first two weeks. This will gradually improve along with any bruising, numbness and tingling. It typically takes 6-12 months for the swelling to fully resolve and at least 12 months for the incision scars to fully mature.

 

 You will need to begin gently walking several times a day the evening of your surgery. This is to promote circulation and prevent blood clots. If you have a personal or family history of blood clots or other bleeding disorders be sure to speak with Dr. Basci.

 

After two weeks, you may begin light cardio activity but be sure to avoid heavy lifting, yoga, pilates and any other intense exercise for 4 weeks. Typically Dr Basci allows necklift patients to resume full activities 6 weeks after surgery.

 

Frequently asked questions

- Am I a candidate for an isolated neck lift?


Having a neck lift is a personal decision. Patients who elect to have a neck lift are typically those who no longer benefit from non-surgical solutions. In most cases a neck lift is paired with a facelift and other procedures to produce a harmonious result with all features rejuvenated in tandem. However, there is a group of individuals with excellent supple skin quality and minimal signs of facial aging (cheek flattening, hollows, nasolabial folds, heavy jowls) who may benefit from a neck lift alone.

If your main concerns are visible banding in the neck, sagging skin, or fullness under the chin, you may be eligible for an isolated neck lift. Dr Basci will analyze your facial and neck anatomy, then assess your skin and tissue quality to determine the appropriate procedure(s) to achieve your goals.

If your primary concern is your jowls, and excess skin along deep marionette lines, of highlighting your jawline a neck lift paired with a facelift or low facelift and facial fat grafting is more likely to give you the desired result. Dr Basci can help you decide which procedure will best suite your needs for a natural long lasting result




- What about liposuction of the neck?


Liposuction is a powerful adjunctive procedure used in conjunction with many different surgeries to help contour soft tissues. In the neck specifically, there is usually very little fat between the skin and the underlying musculature. It’s important to preserve a healthy layer of fat for padding and preventing contour deformities. In some instances when people have excess fatty tissue under their chin, liposuction may be a helpful addition to a neck lift to help debulk and sculpt the desired elongated neck.

While there is no substitute for a neck lift, sometime patients are good candidates for in office neck liposuction to help define their jawline. Dr Basci will assess you goals, anatomy and tissue quality to determine whether neck liposuction with or without a neck lift is the right procedure for you




- How long will my neck lift last?


Longevity of your neck lift result depends on your individual anatomy, the quality of your tissues, the surgical technique and your lifestyle. How the body heals, repetitive cycles of weight gain/loss, sun exposure, and genetic predisposition are unique to each patient and are hard to predict. Typically, a neck lift is said to help you look 10 years younger. You will continue to age after surgery, at a similar rate to how you aged before surgery. The skin and underlying tissue in the neck is quite delicate and gauzy compared to the tissues of the face. Usually the neck is the first place where visible signs of aging return. The timing varies from person to person. The already thin skin continues to lose elasticity with age, repetitive motions and repetitive muscle pull. The tissues eventually lose their ability to recoil and sagging sets in. In patients who undergo neck lift once their skin is already quite inelastic, it is not common to have a small degree of neck skin laxity return in the years after surgery. Many patients elect to have secondary procedures, touch ups, usually of the neck to maintain their result. The timing varies from person to person.




- What about my eyes? My brow? Laser resurfacing? Is it better to address every area at once or separate my procedures?


Not everyone needs every procedure! If Dr. Basci has recommended multiple procedures to address your concerns, it is up to you whether to have them done in one setting or separately. Dr. Basci will discuss how much can safely be performed in one setting and how she recommends timing separate procedures. Some patients prefer having one recovery period. Some patients prefer to separate the procedures for personal, financial or scheduling reasons. Ultimately the choice is yours! Rejuvenating multiple facial features can lead to a more harmonious result and actually make the fact that you had surgery less obvious. For example, a patient who is concerned with signs of aging around her eyes, who opts to start with a neck lift alone may notice the signs of aging around her eyes more than she did before her neck lift.




- I have an important event coming up (i.e. a wedding, a trip to Italy, my son's graduation, my high school reunion), how far in advance should I schedule my facelift?


Dr. Basci recommends having your neck lift a minimum of 4-6 months before a major event like a wedding, TV appearance, family reunion or extended international travel. Ideally, it would be best to have your procedure 1 year before the big day to give your incisions ample time to fully mature. However, in most cases the majority of your swelling should be resolved within 6 months.




- What is can expect during recovery from the elevated neck lift?


Every individual recovery is unique. Patients are typically rest at home for the first 1-2 weeks after surgery, when swelling and bruising are at their maximum. For the first 2-4 days after surgery, you may need pain medication or a sleeping aid. After that, most patients are fairly comfortable and pain-free as they wait for swelling to resolve, using only over the counter medications for pain control. The improvement in neck contour is apparant as early as the morning after surgery. The majority of patients are "restaurant ready" and able to resume social activities with close friends at 2 week when significant swelling and bruising have improved.

We ask that neck lift patients where a gentle compression neck strap 24/7 for the first 2 weeks and then they transition to wearing the the supportive garment only at night. You may shower after your drains are removed, usually within the first several days after surgery. You may apply light makeup after your sutures are removed at your 1 week postoperative visit. You will need to take short gentle walks several times per day during the first two weeks. After that, you may resume light aerobic activity. Most patients may not engage in heavy lifting, yoga, pilates, or other intense activity until 4 weeks after surgery, when they are cleared by Dr. Basci. Most patients are back to full activity by 6 weeks after surgery.

The majority of swelling resolves within the first 3-6 months after surgery. Dr. Basci calls this time "camera ready," because most patients are ready to show off their result and love their rejuvenated look, even though healing is not yet complete. The youthful result continues to remodel and incisions mature for a full 12 months. Dr. Basci recommends waiting for 9-12 months before performing any minor touch ups or refinements.




- What should I bring to my consultation?


It is a good idea to write down any questions you have and bring your written list to your consultation. This way you won’t forget what you wanted to discuss with Dr. Basci!
