Expected Enhancements

A brow lift can change the position and shape of the brow and sometimes even alter the length of the forehead. Whether you are genetically predisposed or you find yourself squinting in the sun and scowling at small font on screens, many women experience descent of their brow with age. As the brow tissue begins to settle below the orbital rim it rests heavily on the upper eyelid. The forehead muscle comes to the rescue by contracting to raise the brows of the eyelids. This effort of the forehead muscle creates horizontal lines that with repeated muscle excursions become permanent landmarks on your skin. Botox is a great quick fix for those unwanted lines, but taking away the forehead’s ability to raise the eyebrows can also leave the brow stranded, perched heavily on the upper eyelid. Many of us don’t even notice we have an eyebrow aging problem because the forehead does such a great job of compensating and lifting the brows out of the way for us.

Most women who seek an attractively arched brow lift fall into two major categories. Either, they’ve recently had a Botox treatment that has unmasked their brow problem or their forehead is no longer able to compensate and elevate their brow effectively due to genetic predisposition and their individual anatomy. The most common complaint is they have trouble applying eyeshadow or mascara keeps rubbing off on the skin where the lashes make contact. The natural conclusion is there’s something wrong with the upper eyelid, and often signs of aging around the eyes is a combination of brow droop and loosened excess eyelid skin. During Surgery

Every person has individual aesthetic desires and eyebrows are especially varied in morphology from person to person and even side to side in one patient. Dr Deniz Basci has advanced training and expertise in brow lift surgery and will design a customized treatment plan based on your needs. There are several distinct surgical approaches Dr. Basci uses to rejuvenate the brow and forehead. Choice of surgical technique depends largely on your individual forehead anatomy and hair quality. Effective results can be obtained by any of the following techniques, pros and cons of each will be reviewed during your consultation

1. Direct Brow Lift
An incision is made just above the brow in a pre-existing deep crease. Lax eyebrow soft tissue attachments are carefully released and the brow is repositioned.

2. Transblepharoplasty
An incision is made in the upper eyelid. The tail of the eyebrow is secured to the deep tissue

3. Hairline Brow Lift
An incision is made along your hairline. The skin is elevated. Lax eyebrow soft tissue attachments are carefully released and the brow is repositioned

4. Coronal Brow Lift
An incision is made from ear to ear 2 to 3 cm behind your hairline. The skin is elevated. Lax eyebrow soft tissue attachments are carefully released and the brow is repositioned.

5. Endoscopic Brow Lift
Limited incisions are made just behind the hairline. A camera is inserted to help visualize meticulous release of lax eyebrow soft tissue attachments and the brow is repositioned.

6. Temporal Brow Lift
When the desired elevation is mostly lateral, towards the tail of the brow, a temporal brow lift is an option. Limited incisions are made behind the hairline on either side of the head. The skin is elevated. Lax eyebrow soft tissue attachments are carefully released and the brow is repositioned. A delicate arch is introduced to brow shape.

Dr. Basci uses meticulous tension free suturing techniques to help your body heal with the most favorable incisions and least amount of damage to hair follicles. In some cases, a drain is placed on one side of the forehead depending on the type of brow lift performed. Patient typically go home the same day unless other procedures are simultaneously being performed that recommend an overnight stay.

Routine Recovery

Just as every individual procedure is distinct, so is every personal recovery. Most patients will go home the same day as their brow lift procedure. You will need a family member of friend to stay with you the first evening for your safety and comfort

You may shower the day after surgery. Avoid curling irons, intensely hot blow dryers, and chemically coloring your hair for 1-2 weeks after surgery, as your scalp may be numb in certain areas. Any non-dissolving sutures will be removed in the office at your first a post-operative visit. You will need to apply a thin layer of ointment to your incisions once a day until they are fully healed. Talk to Dr. Basci about when it is safe to begin lymphatic neck massages and silicone scar treatment.

You should supplement your regular balanced diet with protein shakes or bars for 3-6 weeks after surgery to help improve healing. This is not the time for diets or nutritional restriction. Your body needs vital amino acids and building blocks for healing.

Dr. Basci recommends you sleep with your head propped up on several pillows or semi-upright in a recliner for the first week after surgery to minimize pain and swelling. You should expect to be moderately swollen during the first two weeks. This will gradually improve along with any bruising, numbness and tingling. It typically takes 6-12 months for the swelling to fully resolve and at least 12 months for the incision scars to fully mature.

You will need to begin gently walking several times a day the evening of your surgery. This is to promote circulation and prevent blood clots. If you have a personal or family history of blood clots or other bleeding disorders be sure to speak with Dr. Basci.

After four weeks, you may begin light cardio activity but be sure to avoid heavy lifting, yoga, pilates and any other intense exercise for six weeks. Typically Dr Basci allows brow lift patients to resume full activities six weeks after surgery depending on the nature of their treatment and whether they have undergone other combined procedures.

- Which brow lift technique is best for me?
During your consultation, Dr. Basci will review your goals and analyze your facial anatomy taking particular care to note the length and convexity of your forehead as well as the quality and density of your hair. Each brow lift technique has pros and cons and these will be discussed in detail. Another consideration is whether you are having other facial rejuvenation procedures simultaneously. Often incisions for those adjunctive procedures inform which brow lift technique we choose. Ultimately the choice is yours and Dr. Basci will help guide you to the treatment that is best suited to achieving your desried result
- One of my eyebrows is lower than the other, will you make them even?
Everyone has some variation in brow shape or position from right to left because every face if asymmetric. There is usually a longer and narrower side of the face with a smaller eye and a shorter and wider side of the face with a bigger eye. This asymmetry starts all the way down in your bones. The soft tissues resting on top of an uneven surface will always be slightly asymmetric. It’s no wonder brows are also affected by the natural asymmetry of your structure. In addition, many women introduce new variables in brow shape during brow maintenance.

There will always be a higher and a lower brow. Dr. Basci’s detail oriented approach to brow lifts takes into account existing asymmetries and tailors the procedure to balance the differences when appropriate. Raising the lower brow can increase the distance between the lash line and brow on that side of the face, which some might say is a more obvious difference. While your eyebrow asymmetry should largely improve, it will never be fully corrected because of the uneven bony foundation beneath the soft tissues.

- How long will my brow lift result last?
The longevity of your brow lift result depends on your individual anatomy, the quality of your tissues and the surgical technique. Usually your new youthful brow position is maintained for 5 to 10 years. After surgery the dynamic aging process resumes and over time your brow position will subtly shift.
- What about my eyes? My brow? Laser resurfacing? Is it better to address every area at once or separate my procedures?
Not everyone needs every procedure! If Dr. Basci has recommended multiple procedures to address your concerns, it is up to you whether to have them done in one setting or separately. Dr. Basci will discuss how much can safely be performed in one setting and how she recommends timing separate procedures. Some patients prefer having one recovery period. Some patients prefer to separate the procedures for personal, financial or scheduling reasons. Ultimately the choice is yours!

Rejuvenating multiple facial features can lead to a more harmonious result and actually make the fact that you had surgery less obvious. For example, a patient who is concerned with signs of aging around her eyes, who opts to start with a neck lift alone may notice the signs of aging around her eyes more than she did before her neck lift.

- I have an important event coming up (i.e. a wedding, a trip to Italy, my son's graduation, my high school reunion), how far in advance should I schedule my facelift?
Dr. Basci recommends having your brow lift a minimum of 4 months before a major event like a wedding, TV appearance, family reunion or extended international travel. Ideally, it would be best to have your procedure 1 year before the big day to give your incisions ample time to fully mature. However, in most cases the majority of your swelling should be resolved within 4-6 months.
- What is can expect during recovery from the delicately arched brow lift?
Every individual recovery is unique. Patients are typically rest at home for the first 1-2 weeks after surgery, when swelling and bruising are at their maximum. For the first 2-4 days after surgery, you may need pain medication. After that, most patients are fairly comfortable and pain-free as they wait for swelling to resolve, using only over the counter medications for pain control. The improvement in brow contour as apparant as early as the morning after surgery. In many cases the brow will be over-corrected immediately after surgery to allow for expected settling as swelling goes down. The majority of patients are “restaurant ready” and able to resume social activities with close friends at 7-10 days when significant swelling and bruising have improved.

You may shower the day after surgery, no vigorous scrubbing near incisions in your hairline, but you may alow the incisisons to get wet and for soapy water to trickle over them. Pat dry. AVOID blow drier, curling irons, and any chemical treatments (color, highllights, perms, relaxing oils) for the first 2 weeks after surgery. Your scalp may be numb, putting you at risk for inadvurtantly injuring or burning your scalp. You will need to take short gentle walks several times per day during the first two weeks. After that, you may resume light aerobic activity. Most patients may not engage in heavy lifting, yoga, pilates, or other intense activity until 4 weeks after surgery, when they are cleared by Dr. Basci. Most patients are back to full activity by 6 weeks after surgery.

The majority of swelling resolves within the first 3-6 months after surgery. Dr. Basci calls this time “camera ready,” because most patients are ready to show off their result and love their rejuvenated look, even though healing is not yet complete. The youthful result continues to remodel and incisions mature for a full 12 months. Dr. Basci recommends waiting for 9-12 months before performing any minor touch ups or refinements.

- What should I bring to my consultation?
It is a good idea to write down any questions you have and bring your written list to your consultation. This way you won’t forget what you wanted to discuss with Dr. Basci! If you come up with new questions after the consultation you can always reach out to us via email or schedule another appointment or video consultation.
