Silicone Breast Implant
Silicone Breast implants have a silicone elastomer shell filled with a softer silicone gel that feels very much like natural breast tissue. The silicone gel that fills the shell is a cohesive material, meaning the particles are held together and do not lose their shape when cut through, much like a gummy bear candy that retains its shape when sliced in half. The cohesive gel will not leak or migrate if the implant shell becomes perforated. The newest generation of optimally filled cohesive silicone breast implants offers improved shape retention and is less likely to fold or collapse due to the dynamic movements of everyday life.
Silicone breast implants are FDA approved for breast augmentation in women older than 22 and for any age when used for breast reconstruction. They have been shown to yield excellent aesthetic outcomes and are associated with high patient satisfaction.
Saline Breast Implant
Saline breast implants have a silicone elastomer shell and are filled with saline during surgery, after the empty shell is placed into the breast pocket. This allows the surgeon to fill a customized amount of fluid, offering more flexibility in the final implant volume. Because they are inserted while empty, saline implants require slightly smaller surgical incisions than silicone implants. Saline implants are also less expensive than silicone implants.
However, much like an overfilled water balloon, a saline implant can exhibit rippling along the sides, which may be visible or even palpable on the breast. In the unlikely event that the implant shell is perforated, the salt water will leak out and be reabsorbed immediately, leaving you flat on one side. A short incision is made to access the breast tissue and create a custom-sized pocket for your selected implant, usually under the pectoralis major muscle. The choice of implant position, whether under or over the muscle, is determined by your goals, anatomy, and tissue quality. Most often implant position under the muscle is preferred for a more natural result, with increased implant coverage and a lower risk of capsular contracture.
During your operation, Dr. Basci takes particular care to cause the least amount of tissue trauma and delicately controls any bleeding as she creates the precise pocket. Prior to implant placement, the pocket is copiously irrigated with a specific antibiotic solution to reduce the risk of capsular contracture.
The implant is delivered into the pocket using a no-touch technique to ensure maximal sterility. Finally, the incisions are closed using careful tension-free suturing techniques to help incisions heal in the best way your body produces scar tissue.
Once your incisions are healed, Dr. Basci recommends a comprehensive incisional care routine that includes silicone scar therapies to achieve the best possible incision quality. Choice of Incision
There can be different positions for the incision in breast augmentation surgery, each with its own pros and cons. Dr. Basci will recommend the most suitable incision placement for your specific procedure. If more than one incision site is an option for your anatomy, then the choice is yours!
Breast Fold (Inframammary Fold or IMF Incision)
A 3-5cm incision is hidden in the natural crease under your breast. The IMF approach is a suitable choice for most women. Exceptions include breasts without a well-defined inframammary fold or cases in which the breast fold is being moved lower for symmetry or improved shape. In the vast majority of cases, IMF incisions heal nearly imperceptibly and are well hidden in a natural anatomic crease, helping camouflage the scar.
Around the Nipple Complex (Periareolar Incision)
An incision is made along the lower border of the areola. The periareolar incision is an excellent option for breasts with a smooth areolar border and a distinct color transition from the areolar to the breast skin. In these patients, the residual scar appears to be a natural anatomic border.
The periareolar approach is sometimes beneficial in breasts with a constricted lower pole or tubular breast deformity.
Of note, the periareolar approach may carry a higher risk of capsular contracture and infection because dissection is performed through the breast ducts and gland tissue. It is usually not feasible in breasts with smaller areolar complexes because the incision is not large enough for the implant to pass through.
Underarm (Transaxillary Incision)
A 3-5cm incision is made in the armpit, or axilla. An endoscopic camera is used to guide the dissection of the implant pocket, and the breast implant is delivered through the underarm incision. This technique is best for patients without a well-defined inframammary breast fold or individuals prone to hypertrophic scarring.
The transaxillary approach may carry a higher risk of capsular contracture and infection because dissection is performed through the axillary sweat glands and hair-bearing tissue. Placing a breast implant through the armpit may also have a higher incidence of implant malposition laterally.
Preparing for Surgery
Prior to any surgery, Dr Basci will review your general health history, surgical history, and current medications. You will need to stop taking any medications or supplements that increase the risk of bleeding 2 weeks before surgery and remain off them for two weeks following surgery. Be sure to consult with your primary prescribing physician before discontinuing any medications.
It is important to prime your body with a healthy, balanced diet in the months leading up to surgery. Make sure you have the appropriate protein intake. Protein is vital for healing.
Recovery from Breast Augmentation Surgery
Just as every individual procedure is distinct, so is everyone’s recovery experience. The rapid recovery breast augmentation procedure has one of the most streamlined and consistent immediate post-operative progressions. The rapid recovery breast augmentation technique has a total operative time under one hour and a typical 1-2 hour stay in the post-operative care unit until you are safe to return home. Dr. Basci recommends taking a short nap upon arrival home, followed by a warm cleansing shower, and leaving your waterproof surgical dressing in place. That evening, you should go out to dinner and toast the new you! Resuming regular activities and enjoying your results are essential parts of the healing process.
You can expect mild bruising and swelling, along with moderate chest soreness. Some patients compare the feeling to what you’d experience after an intense pec muscle workout. You may shower the same day as your surgery, leaving your waterproof surgical dressing intact until your 1-week post-operative visit. Avoid sleeping on your stomach and applying direct pressure to your breasts during the first few weeks after surgery. This is to avoid inadvertently causing pain or disrupting your surgical site before it has healed. You may wear a soft, underwire-free bra for gentle support, and a surgical bra will be provided at the time of your procedure.
Any non-dissolving sutures will be removed in the office at your first post-operative visit. You will need to apply a thin layer of ointment to your incisions once a day until they are fully healed, and protect them from sun exposure. It takes 3-4 months for the implants to settle into their ultimate position. You may be instructed to wear a compressive strap in some cases to help maneuver your implant into its final desired position. Bra cup sizes are a fairly unreliable measure of breast volume. That’s because there is no consistency in cup sizes from brand to brand. One woman can easily fit into a so-called B, C, or D cup, depending on where she shops and what strap circumference she chooses. For that reason, it’s confusing and misleading to speak in terms of cup sizes. 3D imaging has been shown to be the most accurate estimate of post-operative breast appearance. To ensure clear communication and realistic expectations, Dr. Basci prefers to plan implant sizes based on your precise dimensions and breast measurements.
- I’d like to enhance my cleavage and bring my breasts closer together. What is the best implant for that?
Your natural breasts sit atop your torso over the pectoralis major muscle. During a breast augmentation, the implant is usually placed partially under this muscle to push forward the overlying breast and support the upper pole. The location of your chest muscles is predetermined by your individual anatomy and cannot be shifted. How close together your breasts are is limited by your anatomy. Having breast augmentation with implants alone will not move your breasts closer to the middle.
Sometimes, this limitation can be overcome by performing breast fat grafting in conjunction with your augmentation. Fat grafting is a technique in which fat and stem cells are borrowed from another part of your body and transferred to your breast. Careful placement of the fat along the top and middle border of your breast may give the appearance that your breasts are subtly closer together.

- When can I buy new bras after my breast augmentation?
For the first 1-2 weeks following surgery, Dr. Basci recommends you wear a soft surgical or athletic bra without underwire. After surgery, you will be placed in a soft surgical bra that clasps in the front. Typically, Dr. Basci asks patients to wait 4-6 weeks before resuming underwire bras, depending on how the implants are settling into the desired position.
- When can I swim after breast augmentation?
Patients may shower the same day as surgery with their waterproof surgical dressing in place, but should avoid submersion in water, such as a bathtub, hot tub, jacuzzi, lake, or pool, for 4-6 weeks. There are many harmful bacteria that your skin protects you from, but while your skin is healing, your protective barrier is compromised. It is important to prevent implant contact with any bacteria both during and after surgery to minimize your risk of infection or capsular contracture. You may enter the water up to your belly button after three weeks. Always protect your incisions with sunscreen to prevent darkening and scarring.
